Immediate Full-Time Occupational Therapist Job Opportunity is available in Aurora, Centennial and Parker, Colorado.
Peak to Peak Home Healthcare, LLC is a licensed and Medicare/Medicaid-accredited home health agency dedicated to providing high-quality skilled home health services in the Denver metro area and surrounding suburbs.
Peak to Peak Home Healthcare, LLC has established a reliable referral network with local hospital systems, rehabilitation and skilled nursing facilities, and physicians’ offices, and our census continues to outgrow our healthcare team. We are currently seeking a highly skilled and compassionate Occupational Therapist to join our growing healthcare team. The Occupational Therapist will provide intermittent in-home occupational therapy services, enabling full clinical autonomy and control over their scheduling and patient outcomes. The Occupational Therapist will never be required to perform start of care admission visits. A pay-per-point system directly impacts compensation when prioritizing volume and efficiency.

Requirements
- Licensed Occupational Therapist in Colorado with an active license in good standing with the Colorado Department of Regulatory Agencies.
- Graduate of a occupational therapy program accredited by the National Board for Certification in Occupational Therapy.
- BLS/CPR certified.
- Comfortable using an EMR platform on a phone or tablet. Paid training is provided.
- Valid driver’s license and a reliable vehicle with current auto insurance.
- Able to work effectively and independently in the patient’s home environment.
- Excellent communication and interpersonal skills.
- One year of experience in a home health or mobile outpatient setting is preferred, but not required.

Duties
- Perform comprehensive in-home assessments and develop customized and individualized plans of care.
- Implement personalized treatments as identified and indicated per the plan of care.
- Maintain effective communication as part of an interdisciplinary team regarding patient care and patient progress, using various communication methods, including the phone, EMR platform, and communication application.
- Maintain accurate and detailed patient records, including patient treatment and patient progress.
- Participate in continuing education and professional development to stay current with occupational therapy standards.
- Maintain patient confidentiality and adhere to HIPAA guidelines.
- Be a key member of an outstanding and supportive interdisciplinary team.
- Perform other clinical and clerical duties as assigned.
