Peak to Peak Home Healthcare, LLC has established a reliable referral network with local hospital systems, rehabilitation and skilled nursing facilities, and physicians’ offices, and our census continues to outgrow our healthcare team. We are currently seeking a compassionate and reliable Certified Nursing Assistant to join our growing healthcare team. The Certified Nursing Assistant is a paraprofessional member of the care team who will provide intermittent in-home services for agency’s patients, enabling full clinical autonomy and control over their scheduling and patient outcomes.

The ideal candidate will have a strong passion for delivering exceptional patient care and working collaboratively with an interdisciplinary healthcare team. This job offers an immediate opening for per diem employment, with assignments thoughtfully selected and matched to the preferred geographical areas. Occasional travel outside of the designated geographical location may be required.

Requirements
- Licensed Certified Nursing Assistant in Colorado with an active license in good standing with the Colorado Department of Regulatory Agencies.
- Graduate of a state-approved Certified Nursing Assistant training program.
- BLS/CPR certified.
- Comfortable using an EMR platform on a phone or tablet. Paid training is provided.
- Valid driver’s license and a reliable vehicle with current auto insurance.
- Able to work effectively and independently in the patient’s home environment.
- Possess ability to adapt to changing priorities.
- Excellent communication and interpersonal skills.
- One year of experience in a home health setting is preferred, but not required.
Duties
- Assist in providing base care to patients, such as bathing, dressing, and grooming.
- Assist licensed nursing staff with patient care, including vital signs, medication reminders, and basic life support.
- Provide emotional support and behavioral health interventions to patients and their families, as needed.
- Observe and monitor patients’ conditions and report any changes to the nursing staff.
- Lift and transfer patients safely and efficiently.
- Maintain patient confidentiality and adhere to HIPAA guidelines.
- Collaborate with interdisciplinary team to develop, implement, and update patient care plans, as required.
- Document patient care in electronic medical records.
- Be a key member of an outstanding and supportive interdisciplinary team by maintaining effective communication regarding patient care using various communication methods, including the phone, EMR platform, and communication application.
- Perform other clinical and clerical duties as assigned.
